Please bring all original documents for scanning.

1. Applicant’s Australian passport.

2. If holding foreign passport other than Australian passport, then provide a photocopy of Australian visa. For New Zealand passport holders, a photocopy of their Medicare card certified by a Justice of Peace (not a nurse, doctor or pharmacist etc.). You can find a list of JPs by clicking the link below.

3. If Indian passport not cancelled then we will need to prepare an Indian Passport Surrender application along with the OCI

4. Original Indian passport (if not cancelled)

5. If Indian passport has already been cancelled, then Indian passport surrender certificate.

6. Cancelled Indian passport.

7. Australian citizenship certificate

8. If any change in name has occured, a photocopy of the change in name certificate - certified by a JP

9. If any change in appearance an additional form must be completed which will also need to be certified by JP

10. If any family members have an OCI card, bring their OCI card.

11. Your employment contract or latest three payslips.

12. Marriage certificate

13. Spouse passport, first page if Australian Passport, first and last page if Indian Passport.

14. Size: 35mm x 45mm for cancellation. 2” x 2” for OCI application. Hard copy and soft copy required for application. If you go to Australia Post or Officeworks then please ask them for an email copy of the digital photo.

Surrender fee for cancelling Indian passport:
